About Extended Horizons Healthcare Limited
We are a provider of care and support services to individuals across all ages, social, cultural, and economic backgrounds who have difficulties resulting from illness, disability and advanced age.
Care is provided in different settings ranging from care-at-home to residential care home settings. we provide bespoke and complete service for those with severe and enduring physical and mental health difficulties including dual diagnosis (illicit substances and mild learning disability) and those who have been through hospital / psychiatric services and have reached a point where they could be discharged from inpatient services. at this point, we provide supported living.
